Blog Post #24: Succeeding on Your Sub-Internship – 4 Rules Every Med Student Should Know

It’s July, and that means thousands of med students are stepping into their Sub-Internships. Whether you’re rotating through the OR, ICU, or clinic, your Sub-I is more than just another clerkship—it’s your audition for residency.

The challenge? Most schools don’t actually teach you how to succeed in it.

As residents who’ve been in your shoes—and who now advise students across all specialties—we’ve learned what separates a “solid student” from the Sub-I who stands out. Below are 4 universal rules to guide you through your rotation, plus a bonus tip that can make all the difference.

Rule 1: Learn the Culture, Fast

Every service has its own rhythm. Some pre-round at 6:00 AM. Others expect notes in by 8:30.

The quicker you figure out what matters to your team, the faster you’ll be seen as indispensable.

Pro Tips:

  • Watch how the intern/junior resident sets the pace—and show up when they do (or earlier).

  • Respect the workflow. Knowing when to stay out of the way is just as valuable as knowing when to help.

  • Walk the fine line between “eager” and “annoying.” It matters.

Rule 2: Present Like a Sub-I, Not a Med Student

This isn’t the time for long-winded monologues. Your presentations should be structured, focused, and tailored to your specialty.

Examples:

  • Surgery: Emphasize pre- and post-op course, plus only the comorbidities relevant to surgical planning.

  • Medicine: Highlight clinical reasoning and management plans.

And don’t just stop at the data. Show initiative by offering a plan—even if it’s not perfect.

Additionally – own your patient. Know their history, pending consults, labs, imaging, and discharge plan. Few things impress more than being the first to update your team about your patient.

Rule 3: Know When You’re Being Evaluated (Spoiler: Always)

Your Sub-I is essentially a month-long interview. From your first email with the coordinator to your daily interactions with residents, nurses, and ancillary staff, you are being observed.

We’ve seen students with great scores and CVs lose opportunities because of negative feedback from staff. Treat every interaction as if it matters—because it does.

Rule 4: Prep Before Day 1

Too many students walk into their Sub-I “cold.” That’s a big mistake.

Spend the week before reviewing relevant anatomy, common cases, meds, workflows, and pimp questions. Ask upperclassmen and residents what they used—every specialty has a gold-standard resource. Make it your bible.

Bonus Tip: Take Initiative (Within Reason)

After the first week, you should be able to anticipate your team’s needs.

Offer to:

  • Help prep the OR or procedural suite – this includes moving patients and obtaining equipment

  • Follow up on labs/imaging, including calling radiology, drawing blood, and running samples down to the lab itself.

  • Draft progress notes or discharge summary

No one expects you to run the service, but filling gaps is what makes you an asset.

Pro Tip: Try to meet with the program director during your first week—and again at the end. This shows initiative, gives you facetime with leadership, and can set you up for a strong letter of recommendation.
